Before you begin the online submission form, please ensure you have all the appropriate information and documents you need to complete the online submission in one sitting. Please note: Only those who have been invited to apply for ordination to the transitional diaconate are eligible for this grant.
You will need the following for your submission:
- Your personal information including name, address, email, and phone
- Information on all student loans including:
- Loan servicer name(s);
- Loan servicer address(es);
- Account name(s) (if available);
- Amount due on each loan
- Information on any personal indebtedness related to your education

Additional Documents Needed
- Document or statement for all student loan servicers detailing the total amount of your student loan debt
- W-9 completed by each student loan servicer
- ACH completed by each student loan servicer
