Have you completed your Annual Parish Survey?
Before you begin the online submission form, please ensure you completed your Annual Parish Survey (APS). Once you have completed the APS ensure you have all the appropriate information and documents noted below to complete the Creation Care Survey online submission in one sitting.
You will need the following for your submission:
- Parish name, address, phone, and email
- Primary contact/person completing survey name and role in parish
- Primary contact/person completing survey phone and email

Energy Audit Status
All parishes and worshipping communities are required by resolution of ECCT’s Annual Convention (2021: Resolution #3, “Care of God’s Creation and Creatures”) to complete an Energy Audit at least every 5 years.
Please be prepared to include the date of your last parish Energy Audit as well as uploading the Audit Report as a PDF.

You will also have the opportunity to respond to the following questions. If you respond “yes” to any of the below, you will also have opportunity to expand on how you have engaged in this work.
- Has your parish incorporated any upgrades to its buildings and grounds to increase energy efficiency?
- Has your parish incorporated the care for God’s creation and creatures in its liturgy and worship?
- Has your parish incorporated the care for God’s creation and creatures into its education and artistic endeavors?
- Has your parish collaborated with other faith organizations or community partners to promote or advocate for the care for God’s creation and creatures?
