Before you begin the online submission form, please ensure you have all the appropriate information and documents you need to complete the online submission in one sitting.

You will need the following for your submission:

  • Your personal information including name, email, and phone
  • Your current parish name

Worship Leader Training

Prior to completing the Worship Leader License application, you must have completed Worship Leader Training.

Please be prepared to provide the date you completed training and the name of your trainer.

Safe Church Training

Prior to completing the Worship Leader License application, you must have completed Safe Church Training in the last three (3) years.

Please be prepared to provide the date you completed training.

Reference

Name and email of your sponsoring parish’s priest or, if your parish is currently without clergy, a warden.

Be prepared to provide your reference contact information while completing your application. A reference form will then be automatically emailed to your reference upon completion of your application.
