Eucharistic Visitor Training
Prior to completing the Eucharistic Visitor License application, you must have completed Eucharistic Visitor Training.
Please be prepared to provide the date you completed training and the name of your trainer.
Safe Church Training
Prior to completing the Eucharistic Visitor License application, you must have completed Safe Church Training in the last three (3) years.
Please be prepared to provide the date you completed training.
Public Records Background Check
Prior to completing the Eucharistic Visitor License application, you must have passed a public record background check in the last five (5) years.
Please be prepared to provide the date your public record background check was completed.
Reference
Name and email of your sponsoring parish’s priest or, if your parish is currently without clergy, a warden.
Be prepared to provide your reference contact information while completing your application. A reference form will then be automatically emailed to your reference upon completion of your application.
